Community Outreach and Primary Care Registered Nurse

We are a non-profit regional Federally Qualified Health Center (FQHC) that has served eastern Connecticut since 1984. Our healthcare professionals provide a full continuum of primary health care, oral health care, behavioral health care and case management services, primarily to a medically underserved population.
We are seeking a Registered Nurse (RN) to assist us in providing our patients with quality, compassionate healthcare. Ideally, you will have prior experience in community outreach setting and teaching experience.
Connecticut Locations: Norwich, CT and Mobile Health Unit (various locations across service area)
Job Summary:
To assess and identify patient needs to ensure that safety and high-quality care is provided through nursing interventions, supervision of staff, education, patient advocacy, and coordination of services. The Community Outreach and Primary Care Registered Nurse will assist in the organizations effort to provide high quality care out in the community to those with limited access to care via the Mobile Health Unit, as well as in the clinic to patients being served at the assigned site(s). This individual will collaborate with medical, behavioral, dental, and systems of care departments to ensure patients are receiving appropriate and necessary care and services based on their needs.
General Duties:
- Provide smooth and timely flow of patients in accordance with Generations Family Health Center guidelines, responding to high volume of patients in short-term time spans
- Reports to assigned Mobile Health Unit location, and help facilitate patient care during active clinic hours
- Responsible for transporting and maintaining clinical supplies, POC testing equipment, vaccines, medications, and other various items necessary to function on the mobile health unit for active clinic hours
- Provide clinical supervision of clinical support staff as assigned and in conjunction with the Nurse Manager
- Demonstrate, understand and perform triage activities for all appointed and walk-in patients and medical phone inquiries; using reasonable and astute clinical judgments in order to expedite the appropriate care and treatment of patients
- Provide health care advice through telephone calls or the Patient Portal
- Develop and provide individualized and group counseling, guidance, and health instruction with the focus on disease prevention and health maintenance
- Provide nursing care during nursing visits based on clinician orders, to meet the needs (physical, mental, and emotional) of patients
- Administer medications, injections, and vaccinations per facility protocols
- Carry out standing orders for medication refills, ordering tests, diagnostic imaging, labs to support the clinicians and notify clinician of positive results as per facilities policy, helping to formulate care plan and communicate all of the above to patients as needed
- Conduct newborn weight visits and lactation education
- Collaborate in the coordination of chronic disease management, including providing follow up visits for basic disease management, including but not limited to: Diabetes, Hypertension, Hyperlipidemia, Asthma/COPD
- Titrate insulin and adjust hypertensive medications based on specific protocols
- Follow up INR visits per facilities policy and protocol
- Follow up after specialty visits to ensure patients are following their current care plan
- Follow up ED visits for specific diagnoses
- Follow up hospital discharge visits with focus on medication reconciliation and follow through and education on care plan and upcoming specialty clinician visits. i.e. effects, complications, and patient usage considerations
- Perform a range of nursing procedures, including dressing changes, suture removal, oxygen administration, and basic wound care
- Teach patients and/or families proper home health care activities, such as giving injections, taking blood pressures, changing wound dressings, etc.
- Provide health materials, resources and education to patients and their families for informational purposes according to need and learning capacity
- Be able to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations
- Participate in the ongoing Quality Assurance and Performance Improvement initiatives. Ensure that all required reports, clinical and administrative documentation is completed in an accurate, timely and complete fashion
- Comply with all federal, state, local, Joint Commission, Occupational Safety and Health Administration (OSHA), DPH, Subcontractor safety and operational regulations, directives and standards
- Maintain universal precautions and infection control practices
- Maintain confidentiality of all information and support patients privacy, rights, and safety, with appropriate documentation in health records of all communication regarding patient care
- Provide coverage for other clinical areas (as required)
- Flexible hours as required as well as potential work on evenings or Saturdays
- Perform various clinical audits to measure outcomes and quality of care, compliance with regulatory requirements, etc.
- Act as a resource to all Generations sites when requested
- Comply with Generations policies and regulatory requirements
- Participate in grant preparation/development/reporting, as well as policy development as appropriate
- Perform other work-related duties as assigned
- Must maintain appropriate attendance
Specific qualifications for the RN role include:
- Current RN license in the State of CT
- Current Cardiopulmonary Resuscitation (CPR) / Basic Life Support (BLS)
- Bachelor of Science in Nursing (BSN) preferred
- Experience in community outreach setting and teaching experience desirable
