St. Paul Elder Services – Kaukauna, WI
Salary Range: $60,000–$70,000 annually

St. Paul Elder Services is seeking a forward-thinking Community Services Coordinator to lead and grow a diverse set of community-based programs designed to support older adults—especially those living with memory loss—to remain engaged, supported, and safe in their homes and community.

This role provides operational and programmatic oversight for Club Gabriel (Adult Day Center), St. Paul at Home (supportive home care), Foot and Nail Clinic (Podiatry), Personal Emergency Response Systems (Medical Alert Technology), and two volunteer-based programs: Elder Match and Help for Hire. As we look toward growth and innovation, this position plays a key role in shaping how community services evolve to meet future needs. This role is also a great connection point to all of the other services within St. Paul Elder Services and will make referrals to additional programs as they see best fits the individual.

The Community Services Coordinator ensures that individuals and families experience seamless, compassionate, and high-quality support across all community programs—while advancing our mission, shared leadership culture, and commitment to dignity, choice, and meaningful engagement.

Essential Responsibilities

  • Provide day-to-day leadership, coordination, and oversight of all community-based programs, ensuring services align with best practices, state regulations, and St. Paul Elder Services policies.
  • Lead program development, implementation, evaluation, and continuous improvement, with a strong focus on quality of life, quality of care, and person-directed outcomes.
  • Conduct associate performance evaluations and assist with associate schedules.
  • Support and collaborate with interdisciplinary team members, volunteers, and community partners.
  • Serve as the primary liaison for internal and external partnerships related to community services and memory support.
  • Oversee program performance through the organization’s QAPI framework, identifying opportunities for innovation and growth.
  • Provide leadership and expertise in services for individuals living with dementia and their care partners. May include completing assessments or meeting one on one to offer support and answer questions.
  • Promote and model St. Paul Elder Services’ core values of dignity, compassion, respect, hospitality, and stewardship.
  • Actively contribute to culture transformation and shared leadership practices.

Qualifications

  • Bachelor’s degree in Therapeutic Recreation, Social Work, Gerontology, Human Services, or a related healthcare field required.
  • Understanding of State Regulations for Adult Day Centers. DHS 105.14
  • Demonstrated experience supporting individuals living with dementia and their care partners.
  • Experience in community-based services, adult day programs, home-based care, or aging services preferred.
  • Strong leadership, communication, and relationship-building skills.
  • Forward-thinking mindset with interest in program development and innovation.
  • Experience working with volunteers and community collaborations is a plus.
  • Ability to work with technology and an interest to advance current system to enhance efficiency
