Complex Discharge Co‑Ordinator Band 6 £41,608 - £50,702 per annum Plus Distant Islands Allowance of £1,406 per annum 37 hours per week Permanent A new and exciting opportunity has arisen for an individual to play a key role in ensuring our patients enjoy as smooth and efficient a journey as possible following admission to the Western Isles Hospital. The successful applicant will work collaboratively and closely with colleagues across health and social care partnerships to both anticipate and plan for the patient journey from admission to discharge. They will act as the main point of contact for patients, their families, and colleagues in tertiary centres seeking to repatriate patients back to the Western Isles hospital for ongoing care. Applicants for the post should have excellent communication skills, along with the ability to problem‑solve and coordinate patient care within the acute setting in conjunction with the wider clinical teams. The successful applicant will be required to register with the PVG Scheme (Protecting Vulnerable Groups Scheme). This post is not eligible for relocation expenses.
