Behavioral Health Counselor

Salary: $63,547–$79,433 DOE (higher possible for highly experienced candidates)

We’re seeking a dedicated Behavioral Health Consultant to support providers as a key member of an Integrated Behavioral Health team. This role focuses on delivering timely, brief interventions, supporting patients with emotional and health-related challenges, and coordinating referrals to internal and external behavioral health services. The ideal candidate is patient-centered, resourceful, and committed to increasing access to behavioral health care in a primary care setting.

Responsibilities

• Partner with the primary care team to support patients with behavioral and emotional health needs

• Provide care management interventions, behavioral health education, and short-term solution-focused support

• Coordinate referrals and link patients and families to community resources

• Maintain accurate documentation in accordance with program guidelines

• Participate in staff training, program evaluation, and quality improvement initiatives

• Uphold confidentiality and organizational policies

Requirements

Requirements to apply

• Master’s degree in social work, counseling, or a related behavioral health field

• Active behavioral health license

• Experience providing behavioral health or case management interventions

• Proficiency with EMR/EHR systems

• Must speak Spanish
