Endeavor Health Services is growing! The Critical Time Intervention (CTI) Team Leader will play a pivotal role in guiding individuals through crucial transitional periods in their lives. You will lead a dedicated team of professionals committed to providing compassionate and effective support, ensuring continuity of care, and empowering the people we serve to achieve stability and independence. Your expertise and leadership will be instrumental in fostering a collaborative and supportive environment that makes a lasting impact on our community. $5,000 SIGN-ON BONUS!

Responsibilities include the following. Other duties may be assigned:

  • Lead and oversee the implementation of the Critical Time Intervention (CTI) model to ensure program fidelity and effectiveness.
  • Supervise and provide guidance to CTI team members, including case managers, an LPN/RN, and Peer Specialists.
  • Develop and maintain collaborative relationships with community partners, service providers, and stakeholders to facilitate client support networks.
  • Conduct regular team meetings to provide guidance and information to all team members.
  • Ensure that all client documentation and records are accurate, up-to-date, and compliant with agency and regulatory standards.
  • Monitor and evaluate program outcomes to identify areas for improvement and develop action plans to address them.
  • Facilitate professional development and training opportunities for team members to enhance their skills and knowledge.
  • Report program performance and outcomes to leadership, ensuring transparency and accountability.
  • Advocate for clients’ needs and rights within the organization and in the broader community.

Qualifications

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Valid driver’s license and reliable transportation.
  • Flexibility to work occasional evenings or weekends as needed.
  • Clinical License in Social Work (LMSW/LCSW), Mental Health Counseling (LMHC), or a related field.
  • Minimum of 3 years in a supervisory role.
  • Knowledge of Microsoft Suite Applications.

Salary starting at $68,000 - $71,000/year depending on education, credential and experience.
