Responsibilities

• Assist individuals with daily living tasks including personal hygiene, meal preparation, and medication reminders.
• Facilitate community integration and social activities to encourage engagement and skill development.
• Implement individualized support plans, documenting progress and any concerns.
• Promote a safe and supportive environment, adhering to all safety protocols and guidelines.
• Collaborate effectively with clients, families, and healthcare professionals to ensure comprehensive care.
• Provide emotional support and encourage autonomy in daily activities.

Requirements

• High school diploma or equivalent required; experience in direct care or related field preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Valid driver’s license and reliable transportation.
• Flexible availability including evenings, weekends, and holidays.
• CPR and First Aid certification preferred or willingness to obtain after hiring.
