Position Summary The Direct Support Professional (DSP) provides direct care, supervision, and skill‑building support to individuals with disabilities and older adults receiving Home‑and‑Community‑Based Services (HCBS). DSPs support individuals in maintaining health, safety, independence, and community integration in accordance with individualized service plans. Description This position operates under Minnesota DHS 245D regulations and requires strict adherence to company policies, service plans, and documentation standards. Services May Include Direct Support Professional (DSP) Services Individualized Home Supports (IHS) – Assistance, skill‑building, and/or supervision Individual Community Living Supports (ICLS) – Support with ADLs, safety oversight, household management, and community engagement Services are provided according to the individual’s approved service plan and authorized service hours. Essential Job Duties Personal Care & Activities of Daily Living (ADLs) Assist individuals with dressing, grooming, bathing, toileting, mobility, and eating as outlined in service plans. Promote dignity, privacy, and independence at all times. Skill‑Building & Independence Support Provide cueing, reminders, and hands‑on assistance to support daily living skills. Encourage independence based on individual abilities and goals. Health, Safety & Supervision Monitor individuals for changes in behavior, health, or safety concerns. Follow emergency procedures and incident reporting requirements. Implement positive support strategies as trained. Household Support Assist with light housekeeping, meal preparation, organization, and maintaining a safe environment. Support routines consistent with the individual’s service plan. Community Participation Support participation in community activities, appointments, and social outings. Assist with transportation when authorized and required. Documentation & Compliance Complete, accurate, and timely documentation of services provided. Report incidents, concerns, or changes promptly according to company policy and DHS requirements. Night Supervision (Overnight Shifts – If Assigned) Remain awake and alert for the duration of the shift. Provide active supervision and observation to ensure individual safety. Assist with nighttime mobility, toileting, reassurance, or care needs. Monitor health and behavioral concerns. Document overnight supports, observations, and incidents. Night Supervision services support individuals with disabilities, cognitive needs, medical complexity, or nighttime vulnerability to ensure continuity of care. Requirements Required Qualifications Must be at least 18 years of age. Ability to work independently and follow written service plans. Strong communication and documentation skills. Compassionate, reliable, and committed to person‑centered care. Ability to maintain confidentiality and professional boundaries. Must successfully pass DHS NetStudy 2.0 background check. Valid driver’s license and reliable transportation may be required, depending on client assignment. Preferred Qualifications Prior experience supporting individuals with disabilities or older adults.
