An esteemed organization is seeking a Director of Case Management to provide leadership and strategic direction for the organization's case management, utilization review, and discharge planning functions. This role focuses on enhancing patient care quality, satisfaction, and operational efficiency. The Director is accountable for planning, coordinating, and evaluating comprehensive patient management programs to align with the organization's mission, vision, and strategic objectives.

Key Responsibilities
  1. Strategic Leadership

    • Develop and implement departmental goals and objectives aligned with the organization's strategic plan.
    • Promote the mission, vision, and values of the organization.
  2. Patient Care Management

    • Coordinate social services and case management functions into patient care and discharge processes.
    • Perform concurrent medical record reviews in compliance with TJC, CMS, and state regulations.
    • Advocate for patients by investigating adverse events and educating staff on resource utilization and discharge planning.
  3. Interdisciplinary Collaboration

    • Facilitate interdisciplinary rounds to optimize care plans and resource utilization.
    • Develop and execute patient care policies and protocols in collaboration with clinical staff.
  4. Performance Monitoring and Improvement

    • Evaluate patient satisfaction and quality of care delivery.
    • Monitor resource utilization from admission to discharge to ensure cost-effectiveness.
  5. Communication and Advocacy

    • Maintain effective communication with physicians, patients, and families to ensure alignment with care goals.
    • Present "denial letters" and handle associated communication as required.
  6. Human Resources and Financial Management

    • Oversee staffing, training, and resource allocation within the department.
    • Support physicians in maintaining cost efficiency and achieving patient outcomes.
Qualifications

Must-Haves:

  1. Bachelor's Degree in Nursing (BSN) or higher.
  2. Active Arizona or Compact State Registered Nurse license.
  3. BLS certification upon hire.
  4. Minimum of 2 years as a Director of Case Management in a critical care hospital.

Preferred Qualifications:

  1. CCM or ACM Certification.
  2. Master's Degree in a relevant field.
  3. 5+ years in case management leadership or upper management in a critical care hospital.
Pay Range:
  • $120K-$140K per year plus additional benefits and sign-on bonus of $20K
