Director of Social Services
Location: City of Industry, CA

Hourly Rate based on relevant experience: $35.00 - $50.00
QUALIFICATIONS: 

1. Master of Social Work (MSW) required. Bachelor of Science (BS) degree in Social Work or Gerontology preferred.  
2. Sufficient verbal and written communication skills to perform the tasks required. 
3. Working knowledge in Minimum Data Set (MDS), care planning, and discharge planning. 

ESSENTIAL JOB FUNCTION: 

1. Identify, evaluate, and document the medically related social, emotional, and personal needs of the resident. 
2. Coordinate with family, community, and staff in meeting the needs of the resident. 
3. Provide crisis intervention for residents and families when appropriate. 
4. Make appropriate arrangements for discharge planning. 
5. Provide in-service education to the staff on the social and emotional needs of the residents and families. 
6. Ensure all required charting and documentation is complete for MDS and Care Plan. 

ADDITIONAL DUTIES: 

1. Oversee duties of Social Services Assistant. 
2. Theft and Loss 
3. Attends staff meetings, Department Head meetings, corporate, and Social Service meetings.  
4. Maintains a high community profile especially with service providers, referral agencies, the medical community, and governmental agencies. 
5. Meets all state and Federal guidelines; as well as, corporate guidelines for Social Service Directors. 
6. Other duties as assigned. 

PHYSICAL REQUIREMENTS: 

1. Occasional pushing and pulling. 
2. Occasional bending and reaching. 
3. Frequent grasping and fine manipulation.
