EAC Network, a leading not-for-profit organization dedicated to empowering over 80,000 individuals through 100 impactful programs across Long Island and NYC, is seeking a motivated Family Communication Specialist to join our Home Base (Intensive) Program. Wage: $32.00 per hour Schedule: 28 hours per week - hybrid schedule. Location: 320 Carleton Ave in Central Islip Core Purpose of this Role The Family Communication Specialist is at the heart of helping families build stronger, and healthier relationships. Key Responsibilities Counseling & Support: Provide individual and family counseling both in-home and in the office to enhance communication and resolve conflicts. Help families navigate crises and strengthen their relationships. Mediation: Mediate and resolve issues within the home environment, focusing on youth and family dynamics. Collaborative Planning: Partner with Case Managers to create comprehensive service plans tailored to each client's needs. Resource Coordination: Establish and maintain connections with local hospitals, treatment programs, and other supportive services to benefit your clients. Crisis Intervention: Offer immediate support and crisis intervention when necessary. Documentation & Representation: Maintain detailed client records, participate in trainings and workshops, and represent the agency in meetings as requested. Transportation: Provide transportation for clients to appointments, enrichment activities, or emergency resources using your own vehicle. Key Requirements Educational Background: Master’s degree in Social Work or Mental Health Counseling; LMSW or LMHC preferred. Experience: At least two years of experience working with families. Skills: Outstanding communication skills, both oral and written. Ability to collaborate effectively with criminal justice, treatment, and mental health systems. Technical Proficiency: Skilled in computer applications, including Microsoft Office, spreadsheets, email, and database software. Licensing: Valid New York State Driver’s License and access to a car.
