About the Role:

The Family Nurse Practitioner (FNP) plays a critical role in delivering comprehensive healthcare services to patients across the lifespan, from infants to the elderly. This position involves assessing, diagnosing, and managing acute and chronic illnesses while emphasizing health promotion and disease prevention. The FNP collaborates closely with interdisciplinary teams to develop individualized care plans that optimize patient outcomes and enhance quality of life. A key outcome of this role is to provide patient-centered care that integrates evidence-based practices with compassionate communication and education. Ultimately, the FNP ensures patient safety and continuity of care in diverse clinical settings, including primary care, urgent care, and specialty areas such as chemotherapy and critical care.

Minimum Qualifications:

  • Current and valid Family Nurse Practitioner (FNP) certification and licensure in the United States.
  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Active Registered Nurse (RN) license in the state of practice.
  • Demonstrated FNP experience in patient assessment, diagnosis, and management across the lifespan.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification.

Preferred Qualifications:

  • Background in Family Medicine.
  • Familiarity with electronic health record (EHR) systems, including but not limited to eClinicalWorks, eCW, and telehealth platforms.
  • Current Indiana Family Nurse Practitioner (FNP) licensure.

Responsibilities:

  • Conduct thorough patient assessments, including history taking, physical examinations, and diagnostic testing to identify health conditions.
  • Develop and implement individualized treatment plans, including prescribing medications and therapies appropriate to patient needs.
  • Provide patient education on disease management, medication adherence, lifestyle modifications, and preventive health measures.
  • Manage acute and chronic conditions such as infections, diabetes, hypertension, and pain through evidence-based interventions.
  • Collaborate with physicians, specialists, and other healthcare professionals to coordinate comprehensive care.
  • Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
  • Maintain accurate and detailed patient records in compliance with regulatory standards and patient safety protocols.
  • Respond to urgent care needs and provide critical care support when required, ensuring timely and effective interventions.
  • Participate in quality improvement initiatives aimed at enhancing patient safety and care delivery processes.

Skills:

Family Nurse Practitioners utilize their proficiency in medical/surgical nursing and urgent care to address a wide range of health issues promptly and effectively. Pain management skills enable the FNP to provide compassionate relief and improve patient comfort during treatment. Patient safety is a continuous priority, guiding the FNP in adhering to protocols and minimizing risks in all care settings. Together, these skills empower the FNP to deliver holistic, evidence-based care that supports patient well-being and fosters trust within the healthcare team.


Four eight hour shifts are scheduled during the week, with one being 12pm-8pm. Saturday rotation in his place with a schedule of 8am-2pm
