Job Description: Family Nurse Practitioner

Position Overview: We are seeking a dedicated and compassionate Family Nurse Practitioner to join our healthcare team. As a Family Nurse Practitioner, you will provide comprehensive primary care services to individuals and families across the lifespan. This role involves assessing, diagnosing, and treating common acute and chronic health conditions, as well as promoting health maintenance and disease prevention.

Key Responsibilities:


  • Conduct comprehensive health assessments, including medical history, physical examinations, and diagnostic tests, to evaluate patients' health status.
  • Diagnose and treat common acute illnesses and injuries, as well as manage chronic health conditions, in collaboration with healthcare teams.
  • Develop and implement individualized treatment plans, including prescribing medications, ordering diagnostic tests, and providing referrals to specialists as needed.
  • Educate patients and families about their health conditions, treatment options, and preventive measures to promote optimal health outcomes.
  • Provide counseling and support to patients on lifestyle modifications, medication adherence, and disease management strategies.
  • Document all patient encounters, assessments, and interventions accurately and timely in electronic health records, ensuring compliance with regulatory standards.
  • Collaborate with other healthcare providers, including physicians, nurses, and allied health professionals, to coordinate care and ensure continuity of services.
  • Participate in quality improvement initiatives, research projects, and continuing education activities to enhance clinical practice and promote evidence-based care.

Qualifications:


  • Master's degree or higher in Nursing from an accredited program.
  • Current certification as a Family Nurse Practitioner (FNP) from a recognized certifying body.
  • Current unrestricted license to practice as a Nurse Practitioner in the state of NY.
  • Minimum of 4 years of clinical experience as a Nurse Practitioner, preferably in primary care or family medicine.
  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.
