Schedule: Full Time

Location: Queens, Brooklyn, Staten Island, and Lower Manhattan (only 50th and below)

Pay: $$ $110k per year + 6,000 Travel Stipend -Negotiable!

QUALIFICATIONS (Must Haves)

  • Currently licensed Registered Nurse in good standing, in the State in which services are provided.
  • Graduate of an accredited school of nursing.
  • At least two-years of experience in an acute care setting, one of which is in a home healthcare setting is preferred.
  • Current CPR certification and must maintain certification.
  • Possess effective communication skills and be able to follow instructions to complete work as scheduled.
  • Active and current driver's license and have a dependable insured automobile. Must be comfortable with significant daily driving to and from client residences.
  • Neat in appearance.

RESPONSIBILITIES

  • Performs the essential activities including assessment, planning, implementation, coordination, monitoring, evaluation, outcomes, and general interventions across case management practice and processes, and makes appropriate referrals to other disciplines.
  • Performs initial evaluation and assessment of client needs and contacts physician for initial/continuing orders
  • Establishes the Care Plan with team and coordinates, implements appropriate alternatives, and makes a determination of expected client outcomes and goals.
  • Assists and instructs client and family members in nursing care required to promote client's health and safety.
  • When assigned, supervises ancillary personnel.
  • Participates in team conferences, as requested, and attends staff in-services programs.
  • Provides Nurse Delegation to appropriate staff and supervises same to assure compliance with the delegation and safety of the clients.
  • Thoroughly explains the Companys mission and value to the client and clients family members, doctors, providers, etc.
  • Explains role and services of the Company, describes clients rights, and provides contact information for complaints.
  • Provides supervision, support, and instruction to RNs, LVNs, LPNs, PCAs, CHHAs, CNAs, Companions.

Job Types: Per diem, PRN, Full-time

Pay: $105,000.00 - $111,000.00 per year
