Minimum Qualifications MINIMUM QUALIFICATIONS:
A bachelor's or higher- level degree in a human services field*
*a degree in a human services field includes the professional disciplines of audiology, community mental health, nursing, nutrition, occupational therapy, physical therapy, psychology, rehabilitation counseling, social work, speech/language pathology, and therapeutic recreation (including concentrations in art, creative arts, dance, drama, music, or physical education.) Also included are related academic disciplines associated with the study of human behavior (e.g., gerontology, human services, sociology, speech communication); or human skills development (e.g., counseling, education, human development.) Examples of non-qualifying degrees include but are not limited to: criminal justice, political science, liberal arts, and business.

Valid unrestricted license to drive in New York State.

Duties Description PRIMARY DUTIES AND RESPONSIBILITIES:
  • Develop Residential Habilitation Plans that address the outcomes as reflected in the ISP as well as the needs and interests of assigned individuals. Plans will address independence, integration, individualization, and productivity.
  • Develop and maintain house-specific and individual-specific Plans of Protective Oversight.
  • Provide oversight to assigned homes and bring issues/concerns to the attention of appropriate team members.
  • Provide guidance to all direct care staff in all areas related to Residential Habilitation.
  • Develop Personal Expenditure Plan for each individual ensuring that individuals' desires and interests are met, and money is budgeted to meet such.
  • Ensure all Residential Habilitation documents are filed in the individual's records, in proper order, and that records are neatly maintained per regulation and policy.
  • Provide input to the House Manager and/or Recreation staff in the planning of community inclusion activities that are appropriate to each individual's interests and abilities.
  • Responsible for maintaining daily clinical records for their assigned case load; will be responsible for acting as part of the ITT to develop a person-centered Residential Habilitation Plan.
  • Complete all required billing submissions and data recording as needed.
  • Other duties to be discussed at time of interview and as assigned.
NOTE: Travel may be required.
