Overview
We are seeking a reliable and compassionate Home Health Admission Registered Nurse (RN) to join our weekend home health care team in the South Park and Brentwood, Pennsylvania areas. This full-time role offers competitive pay, excellent benefits, and the flexibility to work primarily on weekends. The ideal candidate will be an organized, self-directed RN who thrives in providing high-quality, in-home patient care and is comfortable managing admissions independently.

Key Responsibilities

• Conduct Start of Care (SOC) assessments for new home health patients

• Develop and implement individualized patient care plans in collaboration with the healthcare team

• Deliver professional nursing services and ensure continuity of care

• Accurately document all patient interactions and updates in the EMR system

• Manage daily visit schedules and complete required charting within established timeframes

• Provide patient and family education on care plans, medications, and home safety

• Communicate effectively with patients, families, and interdisciplinary team members

Schedule and Shift Details

• Schedule: Friday through Sunday, completing 3 admissions per day

• Includes 1 flexible or administrative day per week (Monday or Thursday preferred)

Qualifications

Required

• Associate degree in Nursing or higher

• Current Registered Nurse (RN) license in the state of Pennsylvania

• Valid Driver's License and reliable transportation

• Minimum of 1 year of nursing experience using electronic medical record (EMR) systems

• Current Basic Life Support (BLS) certification from the American Heart Association or equivalent

Preferred

• Advanced Cardiac Life Support (ACLS) certification

• Previous experience in home health care, patient admissions, or skilled nursing
