We are seeking a skilled and compassionate Home Health Admission Registered Nurse (RN) to join our growing home health care team in Watsontown, Pennsylvania. Key Responsibilities

• Perform comprehensive Start of Care (SOC) assessments for home health patients

• Develop, manage, and implement individualized patient care plans

• Deliver professional nursing services and ensure continuity of care

• Collaborate with the home health care team to support patient recovery and wellness

• Maintain accurate and timely documentation using EMR systems and mobile devices

• Manage daily visits, prioritize tasks, and meet required charting deadlines

• Educate patients and families on care plans, medications, and home safety

Schedule and Shift Details

• Schedule Options: Monday Thursday or Tuesday Friday, 8:00 a.m. 6:00 p.m.

• Workload: Approximately 3 Start of Care (SOC) visits per day

Qualifications

Required

• Associate degree in Nursing or higher

• Current Registered Nurse (RN) license in the state of Pennsylvania

• Valid Driver's License and reliable transportation

• Minimum of 1 year of nursing experience with electronic medical record (EMR) systems

• Current Basic Life Support (BLS) certification from the American Heart Association or equivalent

Preferred

• Advanced Cardiac Life Support (ACLS) certification

• Experience in home health admissions, skilled nursing, or community-based care
