Responsibilities
- Conduct assessments at patient's home to determine patients' condition and needs.
- Provide all the home health aide services at patient's home.
- Develop personalized treatment plans based on individual patient needs
- Visit, Monitor and document patient progress
- Educate patients and their families at patient's home
- Collaborate with other healthcare professionals to ensure comprehensive patient care
