Expert Healthcare is seeking a dedicated and skilled Home Health Occupational Therapist to join our team. In this role, you will provide essential therapy services in the homes of our patients, assisting them in regaining independence and improving their overall quality of life following injury, surgery, or illness. Your responsibilities will include conducting thorough assessments, developing personalized treatment plans, and implementing therapeutic techniques tailored to each patient’s unique needs. We are committed to providing high-quality care in a collaborative and supportive environment, and we value passion, skill, and a patient-centered approach. If you're ready to make a difference in the community, we encourage you to apply! Part time - 15 points/3 days per week, 20 guaranteed hours per week

Alliance, Louisville, Carrollton, Dennison, Urichsville areas as well as Stark, Carroll and Tusc counties  

Responsibilities

  • Perform thorough assessments of patients’ physical, emotional, and cognitive abilities.
  • Design and implement individualized treatment plans in collaboration with healthcare teams.
  • Conduct therapy sessions in patients' homes while adapting interventions based on their progress.
  • Educate patients and family members on therapeutic activities and daily living skills.
  • Document evaluations, progress, and outcomes in accordance with regulatory standards.
  • Adjust treatment plans as needed and communicate effectively with the healthcare team.

Requirements

  • Current and valid Occupational Therapist license in the state of practice.
  • Graduation from an accredited Occupational Therapy program.
  • Minimum one year of experience in a home health setting required.
  • Preferred experience with HomeCare HomeBase charting system.
  • Strong assessment and therapeutic skills.
  • Exceptional communication and interpersonal skills.
  • Ability to work both independently and collaboratively within a healthcare team.
  • Basic Life Support (BLS) certification is required.
  • Schedule: Monday to Friday, Area of coverage is Lincoln City/Newport, Oregon 
  • Must have active OR license prior to start (not compact)
