Firstat Nursing Services an affiliate of Parx Home Health Care is seeking a Home Health Occupational Therapist to provide high-quality and compassionate care to patients in Brevard county Florida. Our agency has been sustaining thousands of clients for over 20 years by providing tailored home health care support services that meet their specific needs. The purpose of this position is to help people achieve their goals of maximum independence by working with patients to develop unique, individualized treatment plans.

The Home Health Occupational Therapist plays an important role in our patient’s lives by helping them to develop or recover the skills needed to perform daily activities in their own home. As a Home Health Occupational Therapist, you will work one-on-one with patients to evaluate their physical, emotional, and environmental needs, create treatment plans, and evaluate progress over time. As a member of our team, you will be working in a supportive environment that values quality care, compassion, and respect for the individual.

Responsibilities

  • Perform patient evaluations and develop individual treatment plans
  • Educate patients and families on appropriate care methods and techniques
  • Utilize adaptive equipment to assist with patient's independence in daily activities
  • Work collaboratively with a team of healthcare professionals
  • Maintain accurate and detailed patient records
  • Monitor and document patient progress and adjust treatment plans as necessary
  • Participate in continuing education to expand knowledge and expertise in the field

Requirements

  • Degree in Occupational Therapy
  • Active license to practice in Florida
  • At least 1 year of experience in home health care or relevant field
  • Ability to work independently and effectively with patients and their families
  • Strong interpersonal and communication skills
  • Knowledge of current Occupational Therapy techniques and practices
