In this vital role, you will provide individualized therapy services in patients' homes, helping them regain their independence and improve their quality of life after injury, surgery, or illness. You will assess patients' needs, create tailored treatment plans, and implement therapeutic strategies aimed at enhancing their daily functioning. Schedule: Monday Through Friday

Responsibilities

  • Conduct comprehensive assessments of patients' physical, emotional, and cognitive abilities.
  • Develop and implement individualized treatment plans in collaboration with other healthcare professionals.
  • Provide therapy sessions in patients' homes, adjusting interventions based on their specific needs and progress.
  • Educate patients and their families about therapeutic activities and strategies to enhance daily living skills.
  • Document patient evaluations, progress, and outcomes in compliance with applicable regulations.
  • Modify treatment plans as necessary and communicate updates to relevant team members.

Requirements

  • Current and valid Occupational Therapist license in the state of practice.
  • Graduation from an accredited Occupational Therapy program.
  • Experience in home health setting preferred.
  • Strong assessment, planning, and therapeutic skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and as part of a healthcare team.
  • Basic Life Support (BLS) certification required.
