• Work Type: Home Health (patient homes)
• Travel: Required (within 20 miles)
• Driving: Mandatory (license + insurance required)

Role Summary

The OT will:

• Evaluate patients in their homes
• Create and implement treatment plans
• Improve independence in daily living activities (ADLs)
• Train patients, caregivers, and family members
• Arrange and adapt equipment for home safety
• Maintain documentation and care records
• Coordinate with physicians, APCs, nurses, and care teams
• Participate in team meetings and discharge planning
• Supervise COTAs and guide Home Health Aides/CNAs
• Act as a patient advocate

Requirements

Mandatory:

• OT license (New Mexico)
• BLS Certification
• Valid driver's license + auto insurance

Preferred:

• 1 year supervised clinical experience (acute/rehab)
• Home Health experience
