Sierra Home Health & Hospice is seeking a Physical Therapist Assistant (PTA) for its comprehensive Home Health Program. The PTA will provide skilled therapy services under the direction of a licensed Physical Therapist in the comfort of patients' homes in Truth or Consequences and the surrounding areas. This is an exciting opportunity for a PTA professional who values independence and enjoys establishing rapport with patients and their families. With branches in both Las Cruces and T or C, Sierra provides comprehensive Home Care and Hospice Programs and is staffed with experienced clinicians to ensure patients receive the highest quality of care.

Responsibilities

  • Perform patients’ evaluations and obtain necessary clinical data.
  • Develop and implement individualized treatment plans based on clinical observations and objectives established by a licensed Physical Therapist.
  • Administer treatment programs as prescribed by the Physical Therapist.
  • Educate patients and their families on proper care and use of equipment.
  • Accurately document treatment plans, progress and outcomes in the electronic medical record system.
  • Communicate effectively with other healthcare professionals and care teams to coordinate patient care.
  • Participate in quality assurance activities and performance improvement initiatives.

Requirements

  • Associate's degree from an accredited Physical Therapist Assistant Program.
  • Current state license as a Physical Therapist Assistant.
  • Minimum of one year of experience in a home health setting preferred.
  • Basic computer skills including EMR documentation and Microsoft Office.
  • Excellent communication and interpersonal skills.
  • Valid driver's license and reliable transportation.
  • Ability to pass a background check and drug screening.
