Schedule Work days: Friday, Saturday, Sunday 8 AM–4 PM or Saturday/Sunday 8 AM–8 PM. Holidays that fall on the weekend are worked. On‑call rotation. Coverage areas include Burlington, Walworth, Waukesha, Jefferson counties. Compensation Hourly pay range: $35.50 – $53.25. Responsibilities Perform all scheduled visits and collaborate with interdisciplinary team members as necessary. Conduct thorough physical, psychosocial and environmental assessments of assigned patients and update the plan of care as appropriate. Follow a care plan that identifies problems (nursing diagnoses) and evaluates outcomes within a set timeframe. Communicate with physicians to confirm and update patient treatment plans and relay changes to the primary registered nurse. Provide ongoing assessment of patient response to treatments, medications, and teaching; apply interventions as appropriate and revise care plans as needed. Schedule patients and case conferences with the primary registered nurse. Assist with discharge planning in accordance with agency standards and utilize interagency, system, and community resources to ensure continuity of care after discharge. Accurately document and update patient records following agency, state, and federal guidelines. Communicate problems to physicians and the primary registered nurse, clearly stating plans, actions, and goals. Adhere to published policies and procedures and stay current with changes or additions. Perform patient positioning and transportation duties that require lifting, pushing, and pulling while using proper technique. Demonstrate the knowledge and skills necessary to provide age‑appropriate care and interpret data specific to each patient's needs. Licensure, Registration, and Certification Registered Nurse license issued by the state in which the team member practices. Basic Life Support (BLS) for Healthcare Providers certification from the American Heart Association; required within 6 months unless otherwise determined. Valid driver’s license issued by the Division of Motor Vehicles or the state of Illinois. Education Associate’s Degree in Nursing. Experience Typically requires 2 years of experience in nursing. Knowledge, Skills & Abilities Proficiency in clinical skills with the ability to work under direction and make sound judgments. Excellent written and verbal communication and interpersonal skills. Ability to develop rapport and maintain positive, professional relationships with patients, staff and physicians. Demonstrated ability to make critical, independent decisions. Strong organization, prioritization, and problem‑solving skills. Ability to multi‑task with frequent interruptions. Proof of a valid driver’s license and a registered, insured vehicle. Physical Requirements Sit, stand, walk and climb stairs for extended periods; move in confined spaces, bend, twist, kneel, squat, and reach overhead. Maintain physical stamina for patient care duties that may span three or more hours. Concentrate effectively despite frequent interruptions. Have adequate vision to read gauges, measuring devices, and written instructions. Use protective clothing when exposed to chemical, hazardous waste or communicable disease. Lift up to 50 lbs. from floor to waist; lift up to 20 lbs. over the head; carry up to 40 lbs. a reasonable distance; push or pull with 30 lbs. of force. Perform a sliding transfer of 150 lbs. with a second person present. Operate all equipment necessary for job duties. Benefits Paid Time Off programs.
