In this critical role, you will provide individualized therapy services to patients in the comfort of their homes, helping them recover and improve their communication, swallowing, and cognitive-communication skills following illness, injury, or surgery. You will assess patients' needs, develop tailored treatment plans, and collaborate with caregivers and healthcare professionals to ensure optimal outcomes. • Conduct comprehensive speech and language assessments to evaluate patients' needs.
• Develop, implement, and modify individualized treatment plans based on assessment results and patient progress.
• Provide therapy services to improve communication and swallowing abilities in patients' homes.
• Educate patients and their families on strategies to enhance communication and functional abilities.
• Document patient evaluations, treatment progress, and outcomes in compliance with regulatory standards.
• Collaborate with other healthcare professionals involved in patient care to ensure a holistic approach to treatment.

Requirements

• Current and valid Speech Language Pathologist license in the state of practice.
• Degree in Speech Language Pathology from an accredited program.
• Experience in home health setting required.
• HomeCare HomeBase charting experience required.
• Strong assessment and treatment planning skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a diverse caseload.
• Basic Life Support (BLS) certification is required.
