Work under the direction and supervision of the Clinical Manager. Support patients with emotional, social, and community resource needs. Collaborate with interdisciplinary team members to ensure high‑quality end‑of‑life care. Qualifications Master’s degree in Social Work (MSW) from a CSWE‑accredited program. Minimum of one year of experience in a healthcare setting. Current licensure in the State of Michigan. Ability to pass criminal background and MVR checks. Completed health statements confirming the ability to perform job duties. Valid driver’s license. Proof of current automobile insurance. Proof of a negative TB test.
