What You'll Do:
- Coordinate and oversee the care of hospice patients, ensuring they receive the highest quality of comfort and care.
- Work closely with an interdisciplinary team to develop personalized care plans that meet the unique needs of each patient.
- Provide emotional support and education to patients and their families, helping them navigate this important phase of life.
- Conduct regular assessments to monitor patient conditions and adjust care plans as necessary.
- Serve as a liaison between patients, families, and other healthcare providers to ensure seamless communication and care delivery.

- A valid nursing license and experience in hospice or palliative care.
