The Care Team Hospice is seeking a full‑time Registered Nurse Case Manager to provide skilled nursing care and coordinated end‑of‑life support to patients in their homes and facility settings while collaborating closely with the interdisciplinary team.
Schedule, Hours & Territory Standard operating hours are Monday through Friday, 8:00AM to 4:30PM
Crown Point, IN, and the surrounding communities within approximately a one‑hour driving radius
RN Case Managers may participate in an on‑call rotation with additional pay
Some branches offer dedicated on‑call staff, resulting in backup on‑call coverage only

Key Responsibilities Conduct comprehensive patient assessments and develop individualized plans of care
Provide skilled nursing care in accordance with hospice standards and physician orders
Coordinate care with physicians, caregivers, and interdisciplinary team members
Educate patients and families on symptom management, disease progression, and end‑of‑life care
Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
Participate in interdisciplinary team meetings and care planning
Support after‑hours patient needs as part of an on‑call rotation when applicable
Qualifications Active Registered Nurse (RN) license
Hospice or home health experience preferred
Strong clinical assessment and care coordination skills
Ability to work independently in the field
Strong communication and documentation skills
Experience using HCHB (Homecare Homebase) preferred
Field Role Requirements Active driver’s license
Ability to pass a criminal background check
Ability to pass a Motor Vehicle Record (MVR) check
Current TB testing in accordance with agency policy
Current CPR certification as required for the role
