Expert Healthcare is looking for a compassionate and skilled Hospice RN to join our dedicated team. In the sensitive and fulfilling field of hospice care, we aim to provide patients and their families with comfort, support, and dignity during the end-of-life journey. As a Hospice Nurse, you will play a vital role in ensuring that our patients receive compassionate and comprehensive care tailored to their unique needs. This position involves conducting patient assessments, managing symptoms, providing emotional support, and collaborating with interdisciplinary teams to enhance the quality of life for patients. At Expert Healthcare, we prioritize a supportive work environment that fosters personal and professional growth. Average of 5 patients per day

Responsibilities

  • Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
  • Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.
  • Manage symptom control and provide skilled nursing interventions as needed.
  • Educate patients and families on end-of-life care and advance care planning.
  • Provide comfort and emotional support to patients and their families during this sensitive time.
  • Document patient care activities accurately in compliance with hospice regulations.
  • Participate in patient care conferences and team meetings to ensure optimal care delivery.

Requirements

  • Current and valid RN license in the state of practice.
  • Strong skills in symptom management and pain relief.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with an interdisciplinary team.
  • Compassionate and empathetic approach to patient care.
  • Basic life support (BLS) certification required.
  • Must have at least 12 months of hospice in home settings experience within the past 3 years.
  • HCHB charting experience required.
  • PointCare Experience Required

Benefits

  • Weekly pay via direct deposit
  • $2,000-$2,100/wk
  • Mileage Reimbursement for patient related travel
