If you want to advance your career and contribute to our vision of creating healthier communities, now and for generations to come, we want you to be a part of our team.Job DescriptionPosition will participate in multi-disciplinary rounds, facilitate patient satisfaction escalations, review quality metrics, conduct quality audits, monitor dashboards, play an active role in case management discharge planning and participate in continuous quality improvement with external referral sources, social workers, discharge planners, case managers, healthcare professionals, families and patients.Minimum QualificationsEducation: Graduate of an accredited School of Professional Nursing Bachelors of Nursing, requiredLicenses/Certifications: Certified in Basic Life Support; Current State of Texas license or temporary/compact license to practice professional nursingExperience / Knowledge / Skills :Minimum of five (5) years nursing experience in an acute care settingTwo (2) years of previous supervisory experience requiredAdditional previous case management, quality and/or clinical documentation knowledge is preferredUnderstanding of quality improvement, performance improvement, change management and patient safety knowledge preferredWorking knowledge of Cerner Electronic Health Record preferredProficient in Microsoft Office, including Excel, Word, PowerPoint and VisioClinical knowledge of assigned clinical conditions and physiological systems to discern the impact of specific indicators on outcomes and to identify potential gaps in clinical care processesAbility to interact with all members of the healthcare team and administrative staff in a professional and courteous mannerInterpersonal skills necessary in order to interact effectively with external referral sources, including social workers, discharge planners, case managers, other healthcare professionals, families and patientsDemonstrate strong skills and a track record for sustained measurement of program outcomes, ability to work independently and with and through others to achieved desired resultsAbility to build relationships and gain alignment for clinical support from superiors and peersAbility to multi-task, prioritize work, pay close attention to details and meet deadlinesExcellent organizational and communication skillsAbility to effectively communicate complex ideas (both conceptual and concrete) by oral and written meansAbility to work independently with little or no supervision and demonstrate excellent problem solving and critical thinking skills when making decisionsPrincipal AccountabilitiesActs as the on-site Program Manager, providing guidance and direction to the care team.Collaborates with, directs, and may delegate to the healthcare team in the attainment of positive outcomes based on data, research, evidence, and professional standards of care.In conjunction with the Medical Director, oversees clinical workflow of the program to ensure quality of care delivered and patient satisfaction is optimal.Serves as super user for EMR.Participates in a systemic, interdisciplinary, and ongoing evaluation of programs, process improvement and desired client-centered outcomes.Leads in-services, educational programs, required department meetings, and continuing education courses to keep team abreast of new developmentsDevelops strong working relationships with external referral sources and internal staff including Admissions, physicians, Outpatient Clinic, Marketing staff and other departments; and ability to adhere to confidentiality policy and procedures. Connect the hospitalists' plan with primary care providers, chronic disease care clinics, insurers, specialists, and post-acute providers when necessary.Implements and evaluates changes in clinical and operational processes.Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.Possible coverage of additional campuses.Demonstrates commitment to caring for every member of our community by creating compassionate and personalized experiences.
