  • Conducts the initial comprehensive assessment of patients upon admission to ensure a thorough understanding of their medical history, current health status, and specific needs.
  • Engages in ongoing and periodic assessments to consistently evaluate the evolving needs of each patient.
  • Monitors and evaluates the effectiveness of services provided, ensuring that they meet established standards and result in satisfactory patient care.
  • Tracks the progress of each patient towards their individual goals, utilizing measurable indicators to demonstrate improvement and areas needing attention.
  • Oversees the critical transitions for patients, carefully assessing all discharge planning needs to ensure a seamless transfer to the next phase of care, whether at home or in another facility.

Requirements

  •         BSN required
  •         New York RN license/registration.
  •          One year of Hospital Care Management experience
  •          Care Management Certification
  •          PRI Certification
