Position Name: Licensed Social Worker - Hospital
Location: Maple Grove, MN 55369
Start Date: ASAP
Employment Type: Full-time, Travel, Contract, Temporary
Contract Length: 13 weeks (Travel Candidates Only)

Pay Rate: $2600 to $2640 Weekly Gross Pay (Non-taxable travel/housing and M&I stipend available for eligible travelers)

Unit: Acute Care Units
Shift: 8 Hours; Days (8:00 AM - 4:30 PM)
Schedule: 40 hours/week guaranteed, 5 shifts per week (Monday-Friday), every 3rd weekend

Position Requirements:
  • Current Social Work Experience: 2+ years - Required
  • Hospital Social Work Experience: 2+ years - Required
  • Hospital Discharge Facilitation Experience - Required
  • Adult and Geriatric Population Experience - Required
  • Prior Travel Experience - Highly Preferred
Education, License, and Certification Requirements:
  • Degree in Social Work - Required
  • LMSW or LCSW or equivalent Social Worker License for the State of MN - Required
  • LBSW License for the State of MN (Accepted at a reduced rate)
  • BLS Certification from AHA - Required
Job Description:

The Social Worker provides services to customers and their families/significant others to help them adjust to illness, injury, or hospitalization.

  • Assists the healthcare team in understanding the significance of social, economic, emotional, and family factors in relation to the customer's illness, treatment, recovery, or end-of-life processes.
  • Facilitates timely, efficient, and appropriate discharge of customers and coordinates the transition to the next level of care, ensuring proper transmission of the plan of care.
  • Identifies needed community services and coordinates these services for customers requiring assistance.
