Model: Field-Forward Hybrid (in-person outreach + limited remote work)

Star Nursing Services is hiring Special Projects Engagement Specialists to support member outreach, engagement, education, and retention across CalAIM Enhanced Care Management (ECM) and Community Supports programs.
This role is essential for connecting with hard-to-reach Medi-Cal members, supporting initial enrollment, assisting with care transitions, and building trust with culturally diverse populations.

Bilingual ability in Cantonese, Mandarin, or Spanish is preferred due to community needs.

What You ll Do

Conduct high-volume outreach to locate, engage, and support CalAIM members
Complete in-person field visits when members cannot be reached by phone
Assist members with ECM enrollment steps, service explanations, scheduling, and follow-up
Build rapport with members who experience medical, behavioral health, or housing challenges
Provide education on Enhanced Care Management (ECM) and Community Supports services
Coordinate with Care Managers, LCMs, Housing Navigators, and health plans
Document all outreach attempts and engagement outcomes in the EMR
Support hospital discharge outreach, transitions of care, and urgent engagement tasks
Participate in local events, community postings, and culturally aligned outreach strategies
Assist with MCP-related reporting, missed contacts, and engagement metrics
Help resolve barriers such as phone disconnection, unstable housing, or communication limitations

Field Expectations

This is a field-forward engagement role:
Multiple in-person contact attempts each week
Travel to member homes, shelters, encampments, facilities, and community locations
Remote work limited to documentation and communication tasks
Requires flexibility to problem-solve in the field

Required Qualifications

Care/Case Management experience required
Minimum 1 year experience in outreach, engagement, social services, CalAIM, health navigation, housing support, community work, or related field
Strong understanding of:
CalAIM
Enhanced Care Management (ECM)
Community Supports
Basic care coordination and referral support
Experience working with vulnerable, diverse, or hard-to-reach populations
Strong interpersonal communication and cultural sensitivity
Bilingual English + (Cantonese OR Mandarin OR Spanish)

Transportation Requirements

Unrestricted active California driver s license
Valid auto insurance
Current vehicle registration
Reliable vehicle for field visits

Technology Requirements

Intermediate proficiency in:
Microsoft SharePoint
Excel
Word
Teams
Adobe applications
Ability to navigate EMR/EHR platforms
Smartphone capability for field communication

Minimum PC Specifications

  • Windows 10 or higher
  • Minimum 8GB RAM (16GB recommended)
  • Intel i5 / AMD Ryzen 5 or higher
  • 256GB SSD or larger
  • Chromebooks not allowed
  • Screenshot of system specifications required during interview

Education

MA, CNA, AA/BA in Social Services, Public Health, Human Services, Behavioral Health, or similar fields
Equivalent experience accepted

Sales Experience is a plus
