RADIAS Health is hiring a Housing Support Specialist to join our House Housing Support Program (HSP) in providing services to individuals experiencing long term homelessness who have mental illness and/or substance use disorders. The Housing Support program helps clients find their own independent housing within Ramsey County and provides supportive services to help them maintain their housing.  

The Housing Support Specialist leads the housing search and provides support and advocacy to overcome barriers affecting housing in the past. They help clients with appointments for housing needs, accessing community resources and basic needs, crisis intervention, goal planning, and assists clients in obtaining Housing Support benefits. Housing Support Specialists will also provide instruction and monitoring of independent living skills to ensure clients have the supports necessary to manage as independently as possible in an apartment setting. 

Hours: Monday - Friday 8 am - 4 pm or 9 am - 5 pm, 40 hours per week

Location: This position works onsite at the Dale Street Apartments in Saint Paul, MN.

Duties and Responsibilities:

  • Provide instruction and monitoring on the identified service areas indicated in the Housing Focused Person Centered Plan and/or Professional Statement of Need
  • Using person-centered approaches, develop a plan to get and/or keep housing
  • Continually monitor client progress and with the clinical team and modifies the treatment plan as appropriate. 
  • The Support Specialist will communicate changes in service needs and recommendations to Program Director, Case Manager, and other people involved with the person's support.
  • Maintain records of progress for each face-to-face meeting and telephone contact with client and other providers involved with housing, such as Economic Assistance and community housing providers. 
  • Assist clients to create a plan to work towards service goals in regular meetings with the client.
  • with the client, and as needed based on the client’s immediate needs.
  • Intervene as necessary for emergencies and/or when he/she assesses that the client is unable to manage his/her housing in a reasonable manner.  Contact to appropriate emergency services, doctors, staff, and emergency family contacts will be made to alert them of intervention planned or taken.
  • Coordinate and cooperate with other agency and community providers to provide the best possible care for the client, including but not limited to landlords and other community resources.  The Support Specialist will complete all necessary Release of Information documents for communication with other providers and keep these current for each client.
  • Maintain records of contact with other providers regarding client care.
  • Meet regularly with Program Director/Mental Health Professional for individual and group reviews of client needs and receive direction for treatment.
  • Assist clients in obtaining and maintaining HSP benefits and necessary assistance in budgeting monthly funds as required by HSP program.  Collaborate with RADIAS and Ramsey County staff in this area. 
  • Must be able to transport clients in the community.

 

Requirements

  • One year of experience (2,000 hours) providing services to vulnerable populations (people experiencing homelessness, unstable housing and/or people with co-occurring substance use disorders and mental illnesses).
  • Must have car, auto insurance, and valid driver's license

Preferred

  • Bachelor's degree in the behavioral health sciences
