Individual Care Plan SpecialistRegularExemptGeneral Description: Responsible for developing interventions and processes to create Individualized Care Plans and Interdisciplinary Alternate Care Team (ICT) meetings to meet Model of Care (MOC) requirements for both the dual eligibility (D-SNP) and chronic conditions (C-SNP) populations.Essential Functions: Develops and establishes interventions aimed at promoting compliance with the goals and scope of individualized care plans (ICP) and Interdisciplinary Alternate Care Team (ICT) according to the Model of Care (MOC). Ensures the updating of changes in the Model of Care (MOC) related to the generation of care plan and Interdisciplinary Alternate Care Team (ICT) discussions. Responsible for all tasks related to the Interdisciplinary Alternate Care Team (ICT) meetings, including but not limited to, selecting cases to be presented in the meeting for discussion, inviting physicians via E-drop and regular mail, coordinating meetings with beneficiaries interested in participating in the meeting with the Interdisciplinary Alternate Care Team (ICT), uploading notes of the population discussed and which in turn are considered as minutes of the meetings, sending letters to facilitate access to the registry of special conditions, among others. Participates in Alternate Interdisciplinary Care Team meetings as convened to present approval of document revisions related to the process of generating care plans and/or ICT. Responsible for the development of minutes evidence for the alternate ICT meeting. Analyzes reports received to ensure compliance with established requirements. Prepares periodic reports to ensure compliance with ICT and ICP according to the Model of Care (MOC). Presents performance metric reports, related to the Interdisciplinary Alternate Care Team (ICT) and Care Plans, to the immediate supervisor. Monitors compliance with interdepartmental operational processes to ensure adherence with the generation of care plans as required for the various initiatives within the individualized care plans. Responsible for generating tickets to MOE or IT when identifying inconsistencies in processes related to Alternate ICT and ICP. Support in the updating of policies and procedures, processes and/or new Medicare regulations. As required, establishes telephone communication with assigned enrollees to update individualized care plan goals. Periodically reviews the process of recording standardized notes in the TruCare file to ensure reliability scheduling. Must comply fully and consistently with all company policies and procedures, with local and federal laws as well as with the regulations applicable to our Industry, to maintain appropriate business and employment practices. May carry out other duties and responsibilities as assigned, according to the requirements of education and experience contained in this document.Minimum Qualifications:Education and Experience: Bachelor's degree in Nursing. Health Services Administration or related field. Minimum of two (2) years of experience in clinical areas, including documentation or analysis of health conditions.Proven experience may be replaced by previously established requirements.Certifications / Licenses: N/AOther: Advanced knowledge in at least four tools - Access, Excel, Power Point and Word.
