Intake Coordinator
**Key Responsibilities:**
- Conduct initial client assessments to determine care needs and create customized care plans.
- Onboard new clients, including completing initial assessments at clients home.
- Familiar with HHA Exchange System, Word, excel, and effective communicator in writing.
- Coordinate caregiver assignments, ensuring the right match between caregiver skills and client requirements.
- Schedule and manage caregiver shifts, handling day-to-day scheduling changes and addressing any issues promptly.
- Maintain clear and effective communication with clients, their families, and caregivers to ensure the highest quality of care and client satisfaction.
- Conduct regular client follow-ups and reassessments to monitor the effectiveness of care plans and make necessary adjustments.
- Collaborate with healthcare professionals, including nurses, therapists, and physicians, to provide comprehensive care for clients.
- Ensure compliance with all relevant regulations and agency policies.
- Assist in the recruitment and orientation of new caregivers.
- Keep accurate records of client care plans, caregiver schedules, and client interactions.
- Focus on client home care management and clientele growth.
Experience:
- Associate Degree Preferred
-Hha exchange experience
-Bilingual (Spanish/English)
- Proven experience in home care coordination or a similar homecare role.
- Strong knowledge of home care regulations and best practices.
- Excellent organizational and communication skills.
- Compassionate and empathetic attitude toward clients and their families.
- Proficiency in scheduling software and Microsoft Office Suite.
- Ability to work independently and as part of a team.
- Strong problem-solving and decision-making skills.
- Reliable transportation and a valid driver's license.
