Position Summary
The Intake Specialist / Care Coordinator serves as the first point of contact for all incoming patient referrals in a fast-paced, mobile wound care organization. This role is responsible for managing referrals from multiple channels (phone, fax, email, EHR integrations), ensuring accurate data entry, verifying insurance eligibility, and coordinating timely patient scheduling with clinical teams.
The ideal candidate thrives in a high-volume healthcare environment, is detail-oriented, and understands the urgency and complexity of wound care coordination across Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), Home Health, and other post-acute settings.
Key Responsibilities
Referral Intake Processing
- Receive and manage referrals via:
- Phone calls
- Electronic fax systems
- Secure email
- EHR/portal integrations
- Accurately enter patient demographics, insurance, and clinical details into the EHR
- Ensure all required documentation is complete prior to scheduling
Insurance Verification
- Verify patient insurance eligibility and benefits
- Identify authorization requirements and initiate requests as needed
- Communicate coverage limitations to internal teams
Care Coordination & Scheduling
- Coordinate patient visits with mobile providers based on:
- Geography and route optimization
- Clinical urgency
- Facility preferences
- Communicate scheduling updates with facilities, patients, and providers
Communication & Relationship Management
- Act as liaison between:
- Referral sources (SNFs, ALFs, hospitals, home health agencies)
- Internal clinical teams
- Billing and administrative departments
- Provide timely updates on referral status
Documentation & Compliance
- Maintain accurate, timely, and compliant records in accordance with HIPAA regulations
- Ensure all intake processes align with company SOPs and payer requirements
Process Improvements
- Identify inefficiencies in intake workflows and recommend improvements
- Support implementation of automation tools and AI-driven intake solutions (as applicable)
Required Qualifications
- High school diploma or equivalent (Associate’s or Bachelor’s preferred)
- 2+ years of healthcare intake, care coordination, or medical administrative experience
- Experience working with:
- Electronic Health Records (EHR)
- Insurance verification systems
- High-volume referral environments
- Strong understanding of:
- Medicare, Medicaid, and commercial insurance basics
- Excellent organizational and multitasking skills
Preferred Qualifications
- Experience in wound care, home health, or post-acute care settings
- Familiarity with mobile healthcare delivery models
- Knowledge of prior authorization workflows
- Experience handling electronic fax platforms and referral portals
