Provide accurate information about our home care programs. Guide clients step‑by‑step through enrollment paperwork and documentation. Maintain detailed and organized records of all client interactions in our CRM system. Collaborate with internal teams to ensure a smooth transition from patient intake to active service. Follow up with clients to confirm completion and satisfaction. Qualifications Excellent communication and interpersonal skills. Empathetic and patient, with a strong commitment to helping others. Ability to explain complex information in a clear and understandable manner. Strong organizational skills and attention to detail. 2 Years previous experience in Home Care. Familiarity with home care services and the different MLTC Programs.
