Sunrise Senior Living - - Responsibilities: Review, read, notate, and initial Daily Log to document and learn about residents; Participate in the development of the ISPs and monthly updates; Responsible for a designated group of residents during the shift, knows where residents are, and physically checks on them throughout the shift; Attend daily Cross Over meetings by the lead care manager; Assist with daily living activities and life enrichment activities (ADLs) and Encourage/Assist residents to participate as indicated on ISP
