Licensed Practical Nurse (LPN) will provide direct patient care in a long-term care setting, delivering age-appropriate, patient-centered nursing services. Responsibilities include medication administration, patient monitoring, documentation, assistance with ADLs, care coordination, and collaboration with interdisciplinary healthcare teams. Practice will align with clinical standards, safety protocols, and regulatory guidelines.

• Deliver direct nursing care in LTC setting

• Administer medications and treatments

• Monitor patient conditions and report changes

• Assist with ADLs and daily care needs

• Maintain accurate documentation in EMR

• Collaborate with nursing and care teams

• Follow infection control and safety protocols

• Support patient safety and quality standards

• BLS/BCLS
