Pay Rate: $22.00 $24.00/hr + mileage reimbursement
Position Type: 12-Month W2 Contract (renewal based on performance, project outcomes, and health plan continuation)
Schedule: 9:00 AM 5:30 PM PST, Monday Friday
Model: Field-Forward (multiple in-person visits weekly)
Residency Requirement: Must currently live in either Santa Cruz County or Mariposa County to qualify

Star Nursing Services is hiring Special Projects Housing Navigators/Care Coordinators to support CalAIM Enhanced Care Management (ECM) and Community Supports Housing Services in Santa Cruz and Mariposa counties. This role supports members facing housing instability, chronic conditions, and complex barriers to care.

These positions are high-touch, in the field, and essential to the delivery of Community Supports Housing Navigation, Housing Deposits, Housing Tenancy and Sustaining Services (HTSS), and other CalAIM services.

What You'll Do

Provide direct Housing Navigation for CalAIM members, including locating units, coordinating applications, supporting move-ins, and addressing housing barriers
Conduct frequent in-person visits including home visits, housing site visits, and community meetings
Deliver Community Supports services including Housing Navigation, Housing Deposits, Housing Tenancy & Sustaining Services (HTSS), and support with transitions
Assist members with documentation, applications, landlord communication, and stabilizing housing
Complete assessments, support plans, and ongoing housing coordination
Work closely with Enhanced Care Management (ECM) teams, housing authorities, shelters, and community agencies
Support escalations and collaborate with Managed Care Plan (MCP) partners
Document all services thoroughly in the EMR and meet all MCP timelines
Support audit readiness, findings review, and corrective action plan tasks
Provide culturally responsive, bilingual support to Medi-Cal members

Important Field Expectations

This is a field-forward role requiring frequent in-person engagement:
Multiple in-person visits weekly
Daily travel throughout Santa Cruz or Mariposa County
Remote duties limited to documentation and administrative tasks

Required Qualifications

Care/Case Management experience required
Minimum 1 year experience in housing navigation, case management, social services, public health, or CalAIM ECM/CS roles
Strong understanding of:
CalAIM Community Supports
Enhanced Care Management (ECM)
Housing Navigation & Tenancy Services
County-specific housing resources and social service systems
Experience supporting vulnerable populations (behavioral health, chronic illness, housing instability)
Experience with audits, CAP tasks, documentation standards
Bilingual Spanish + English required
Must reside in Santa Cruz County or Mariposa County

Transportation Requirements

Unrestricted active California driver's license
Valid auto insurance
Current vehicle registration
Reliable vehicle for county-wide travel

Technology Requirements

Intermediate experience with:
Microsoft SharePoint
Microsoft Excel
Microsoft Word
Microsoft Teams
Adobe (PDF editing, forms, e-sign tools)
EMRs for CalAIM documentation

Minimum PC Specifications

  • Windows 10 or higher
  • Minimum 8GB RAM (16GB recommended)
  • Intel i5 / AMD Ryzen 5 or higher
  • 256GB SSD or larger
  • Chromebooks not accepted
  • Screenshot of computer specs required during interview

Education

AA/BA, or equivalent experience in case management, housing navigation, social services, public health, or related CalAIM functions
