Position: Full-Time
Schedule: Monday to Friday, 9 AM – 5 PM

Primary Duties:

• Assist individuals facing mental health and social barriers to improve their well-being.
• Collaborate with healthcare professionals to ensure seamless care coordination.
• Establish partnerships with local agencies to connect patients with community resources.

Salary: $90,000 - $110,000 per year, plus comprehensive benefits.

Requirements

Qualifications:

• Master’s degree in Social Work or a related field.
• Active LCSW license.
• Proven experience working with aging populations.
• Strong skills in clinical evaluations and care planning.
• Effective communication and team collaboration abilities.
