Salary: $110,000 - $130,000 per year, plus full benefits.

Key Responsibilities:

• Develop and implement individualized treatment plans that address the specific needs and goals of each client..
• Deliver educational resources and ongoing support to patients and their caregivers, fostering better understanding and management of health conditions.
• Act as a key liaison between patients, caregivers, and healthcare professionals, ensuring clear communication and coordinated care.
• Understand each patient’s unique needs and preferences to provide a seamless and tailored experience, promoting adherence to treatment plans.
• Keep thorough and up-to-date records, including progress notes, treatment plans, and assessments, to ensure compliance with clinical standards and regulations.
• Offer confidential, non-medical counseling to patients experiencing chronic pain, helping them process their experiences and improve their well-being.

Requirements

• Master'sdegree in Social Work from an accredited institution
• Got a valid and active CA state license to practice as a Licensed Clinical Social Worker (LCSW)
• Minimum of 2 years of clinical experience to the table, with a focus on providing psychotherapy services
• Show off your knowledge of solution-based psychotherapy interventions and techniques.
