Program SummaryIn the Adult PHP and IOP, we focus on creating a therapeutic environment where people can learn strategies for coping and improve their functioning. Each client’s treatment plan is centered on group therapy which allows for sharing of experiences while gaining support and validation. When people need intensive, comprehensive treatment for psychiatric and chemical dependency issues but do not need inpatient or residential hospital care, Natchaug offers intensive, structured clinical treatment in programs that may include up to six hours of treatment a day, five days a week. These intensive treatment programs can help prevent the need for hospitalization. They can also help people make the transition to home after inpatient or residential treatment. Natchaug brings this clinical care closer to people’s homes by offering programs throughout Eastern Connecticut. Job SummaryLicensed Behavioral Health Clinicians at Natchaug works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Adult Outpatient department. Clinical services include individual, group and family therapy.  Maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics. This is a regular 24 hour/week position, (3) days/week; Monday, Wednesday and Thursday; 12:30PM - 9:00PM and eligible for benefits.  This position requires ability to float to other site locations when needed. Additional compensation offered.  Key responsibilities of Licensed Behavioral Health Clinician: Conduct a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate. Provide direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning. Refer patient to appropriate levels of care and available resources that promote the individualized care plan.  Maintain timely patient-centered documentation that reflect the patient and client systems’ pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies.  Independently able to provide clinical social work interventions to all patient populations. Provide social work field supervision to second year social work students. ***Behavioral Health Clinicians are eligible for a Sign On Bonus up to $1,500.00, in accordance with program guidelines*** Qualifications Qualifications - External Master’s degree in social work, or Counseling, or Marriage and Family Therapy or Related Human Service Field. Previous experience in a healthcare setting required, preferably working with a chemical dependency population, adults preferred. Current Connecticut LCSW, LPC, LMFT, or LADC or Psychologist. Excellent communication skills, with the ability to establish relationships within and outside their entity Is cognizant of health-related social needs and cultural competence. Recognizes and addresses health disparities-inequalities and injustices directed toward clients, related to access to care and provision of health services.
