The primary functions of an LPN involve assessing, planning, implementing, and evaluating the care of all patients, regardless of age, during their shift. Responsibilities include meeting the established standards of skilled nursing practice, collaborating with the care team, participating in interdisciplinary care, advocating for patients, and effectively providing person‑centered care and services. Roles and Responsibilities Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with facility policy. Takes patients' vital signs (respiration, temperature, blood pressure, heart rate) and reports changes in patients'/residents' conditions. Practices ethically and professionally within scope of practice and advocates for patients. Maintains clinical competence specific to the care area, including required education within the stated timeframe. Coordinates care with the interdisciplinary team. Documents appropriately and thoroughly in EMR/Point Click Care. Informs other nursing personnel and the responsible RN of changes in patients’/residents’ conditions or other pertinent observations. Participates in unit councils and staff meetings. Weekend rotation required. Required Skills and Experience Graduate of a Nursing Program. Current NYS LPN License. CPR/BLS certification. Preferred Skills and Experience Prior patient care and customer service experience. Physical Requirements May require lifting (up to 50 lbs.), may require sitting or standing for long periods. Must be able to physically operate the equipment required for the job. Location and Travel Requirements Onsite. Pay Range Disclosure $24.04 to $32.21 per hour.
