Our client is seeking a Licensed Practical Nurse to join their Hospice care team in Carlinville, IL. This full-time role offers a rewarding opportunity to provide compassionate, end-of-life care in patients' homes and other local care settings. You will collaborate with the interdisciplinary team to deliver high-quality care plans, support patients and families, and ensure timely documentation.

Responsibilities include providing and managing patient care plans, supporting patients throughout the local area, completing accurate charting within deadlines, managing your own schedule, and driving to patient locations as needed.

Schedule: Work schedule and shift expectations will be discussed during onboarding.
Travel: Local travel required within the assigned territory.

Required qualifications:

• Active Licensed Practical Nurse (LPN) license in the State of Illinois

• Associate degree or higher in Nursing

• Minimum of two years of hospice experience preferred

• Proficiency with EMR systems such as TruChart

• Valid driver's license, reliable transportation, and auto insurance
