Position Summary
We are seeking a dedicated and reliable Licensed Practical Nurse (LPN) to join our care team. As an LPN at St. Johnsbury Center for Living, you will play a vital role in delivering hands-on nursing care, monitoring residents’ health, and supporting their overall well-being. Key Responsibilities
Provide direct nursing care to residents in accordance with care plans and physician orders.
Administer medications and treatments accurately and safely.
Monitor residents’ health status and promptly report changes to the RN or physician.
Document resident care and maintain accurate records in compliance with regulations.
Assist with daily activities and support residents in maintaining their highest level of independence.
Collaborate with the nursing team, residents, and families to ensure a supportive, caring environment.
Adhere to all infection control, safety, and facility policies.
Qualifications
Current Licensed Practical Nurse (LPN) license in Vermont (or ability to obtain).
Strong clinical, assessment, and communication skills.
Compassionate, patient, and dedicated to resident-centered care.
Ability to work effectively as part of a team.
Prior long-term care or skilled nursing experience preferred, but not required.
