Shift Notes: 12-hour shifts, 36 hours per week, every other weekend required
Duration: Contract

Position Overview

Universal HCR is seeking a dedicated Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to provide compassionate care to residents in a Long-Term Care (LTC) setting in Iron River, MI. This is an excellent opportunity for skilled professionals passionate about enhancing the well-being of individuals in a collaborative environment.

Key Responsibilities

  • Resident Observation: Monitor residents for changes in mental and physical well-being; communicate observations to the Supervisor and notify physicians as necessary.
  • Shift Reporting: Provide detailed status updates during shift changes and maintain accurate documentation.
  • Medication Administration: Accurately receive, transcribe, and carry out physician orders; administer medications and treatments with proper documentation in a computerized system.
  • Wound Care: Perform wound care, including managing wound vacs.
  • Unit Operations:
  • Coordinate resident appointments, therapy sessions, and activities.
  • Ensure proper documentation of ambulation lists, nutritional charts, and vitals.
  • Monitor infection control practices and maintain a clean, hazard-free environment.
  • Resident Management:
  • Facilitate admissions, discharges, and transfers while ensuring all documentation and notifications are completed.
  • Address resident and family concerns, maintaining open and effective communication.
  • Collaboration: Support and guide LNAs, ensuring accurate documentation and adherence to care plans.
  • Regulatory Compliance: Adhere to state and federal regulations for medication storage, usage, and documentation, including controlled substances management.
  • Administrative Duties: Complete 24-hour reports, manage new orders, and maintain consistent communication with supervisory staff.

Qualifications

Education & Experience:

  • Active LVN/LPN License in the state of Michigan Required
  • BLS Certification Required
  • Long-term care experience Preferred

Skills & Knowledge:

  • Proficient in medication administration and computerized documentation.
  • Skilled in wound care and infection control practices.
  • Excellent organizational, communication, and problem-solving skills.
