Responsibilities:
- Provide direct skilled nursing care
- Tracheostomy care and suctioning
- G-tube feeding and medication administration
- Wound care management
- Medication administration and documentation
- Monitor and report changes in condition
- Collaborate with interdisciplinary team
- Maintain compliance with CDPH regulations
- Current California LVN license (in good standing)
- Experience with trach and G-tube preferred
- Wound care experience a plus
- Strong documentation skills
- CPR/BLS certified
