Job Summary:

The Licensed Vocational Nurse (LVN) is responsible for providing direct resident care by following the plan of care, providing treatments, and working with the clinical staff and other health care team members. The LVN can be assigned duties as a treatment nurse, medication nurse, or charge nurse in the absence of an RN.

Essential Duties and Responsibilities:

  • Direct patient care involves conducting assessments and treatments within established protocols. This includes administering medication and being highly knowledgeable in orthostatic vital signs, nursing assessments, height/weight measurements, and adhering to the patient's schedule.
  • Providing accurate and timely documentation consistent with the care provided is essential.
  • Administrative duties include completing medical forms, reports, evaluations, and charting.
  • Communication with the physician and Director of Nursing about patient status and care is crucial.
  • Monitoring and reporting adverse reactions to medication and treatment are necessary responsibilities.
  • Handling urgent and emergent situations according to company policies and procedures is required. Assisting in the admission and discharge of patients is part of the role. Providing information to residents and family members on prevention techniques as well as ways to maintain or improve general health and psychosocial status is important.
  • Maintaining confidentiality of all patient care information, including protected health information, must be always upheld.
  • Performs other duties as assigned by the Director of Nursing.

Requirements

Minimum Qualifications:

  • Must possess a current and unencumbered active license to practice as an LVN in the State of California.
  • Must comply with accepted professional standards and practice, and guidelines that pertain to long-term care.
  • Ability to confront behavioral changes, identify a crisis, and recognize when additional assistance is needed.
  • Must be willing to work 12-hour shifts on Mondays, Tuesdays, and Wednesdays.
  • Ability to demonstrate respect for cultural and lifestyle diversities of patients and staff.
  • Must present a professional demeanor.
  • Must demonstrate initiative, good judgment, interpersonal skills, and leadership skills.
  • Must be highly organized and demonstrate strong technical skills.
  • Ability to multi-task and work effectively in a high-stress and fast-moving environment.
