Job Description :
The Licensed Masters of Social Work (LMSW) in Home Health provides skilled social work services to patients in East Portland, OR, and surrounding areas. This role involves managing a patient caseload with a focus on delivering comprehensive support and care coordination in a home health setting. The LMSW will conduct patient assessments, collaborate with interdisciplinary teams, and ensure compliance with clinical documentation standards. Strong communication skills and the ability to work independently with a flexible schedule are essential.
Responsibilities:- Conduct psychosocial assessments and develop individualized care plans for home health patients.
- Provide case management services, including patient advocacy and resource coordination.
- Collaborate with interdisciplinary teams to support patient care goals and discharge planning.
- Document patient encounters and maintain accurate records using EPIC Remote Client and Rover/Oasis systems.
- Manage a typical caseload of up to 6 patients per day, meeting productivity standards.
- Ensure compliance with facility policies, including COVID-19 vaccination requirements.
- Maintain effective communication with patients, families, and healthcare providers.
- Travel to patient homes within a 60-90 minute maximum drive radius daily, utilizing own transportation.
- Active Licensed Masters of Social Work (LMSW) credential.
- Minimum 1 year of skilled home health experience.
- Basic Cardiac Life Support (BCLS) certification.
- Valid driver's license, car insurance, and reliable automobile for patient visits.
- Proficiency with EPIC Remote Client and Rover/Oasis documentation systems.
- Primary series COVID-19 vaccination plus one booster dose as required by facility.
- Experience with OASIS documentation and home health regulatory requirements.
- Strong interpersonal and communication skills for patient and team collaboration.
- Ability to manage multiple patients efficiently within assigned productivity standards.
- No weekend or on-call requirements.
- Facility does not accommodate 4x10 or 3x12 shift patterns.
- Traveler may be called off up to one shift per pay period.
- Must confirm Return to Office (RTO) policies with hiring manager.
- Must have own transportation; patient visits are grouped to minimize drive time.
- 8-hour day shifts only.
- Standard caseload with productivity expectations of up to 6 patients per day.
- Block scheduling is not accommodated.
